If you are more than 30 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply. We will do our very best to reschedule your service for another time that is convenient for you.
In the event of a true, unavoidable emergency, your cancellation fee will be made into credit which you can apply to future services.
I have the right to reschedule the meeting as long as I give a 12 hour notice.
Confidentiality Statement
Professional ethics and the laws prevent me from telling anyone else what we discuss in treatment unless you give me written permission to do so. These rules and laws are the ways our society recognizes and supports the privacy or “confidentiality” of therapy.
There are times when the law requires me to reveal certain information. When the safety and welfare of yourself or another person may be in danger the law requires disclosure.
E.g., 1)
If you or other persons are in danger, the law requires disclosure or; if you are threatening serious harm to another person, I am required to try to protect that person. This may result in notifying the police. We are also required to notify the person being threatened.
E.g., 2)
If you seriously threaten or act in a way that is very likely to harm yourself. If you indicate that you have a plan to kill yourself, I may have to seek a hospital for you, or call a family member or other person who can help protect you. If such a situation arises, I will discuss the situation with you before we proceed, unless there is a very strong reason not to.
There are a few other things you must know about confidentiality and your treatment: I may consult with other professionals about your treatment only if I believe a referral is necessary. In this case I will always get consent from you first and the referred professional is also required by professional ethics to keep your information confidential.
I am required to keep records of your treatment. You have the right to review these records.
Any emails I receive from you and any responses sent to you once treatment has started to become a part of your legal record. Please do not send information requiring immediate reply over email, as email is not checked after business hours (after 5 p.m.) or on holidays and weekends.
